Use this as a starting list. Keep private medical details out of public forms and only share the minimum necessary information.

Checklist

  • Current appointments and next visit dates
  • Primary contacts: family, doctor office, pharmacy, helper, transportation
  • Documents to locate: ID, insurance card, discharge note, current med list, care instructions
  • Open tasks: owner, due date, source, status
  • Permission state: who can see what and why
  • Weekly handoff: what changed, what is blocked, what needs a decision
